In Help Wanted 2, Lefty returns as the main antagonist of the "First Aid" game mode. Players must provide three rounds of medical treatment to Helpy while avoiding Lefty's attacks.

Lefty is also a salvageable and buyable animatronic in the game. He causes the most damage of any animatronic and is known for his distinctive appearance. Lefty's head can be seen in the "Special Delivery" section of the game.
